As required by section 3221(h) of the CARES Act (which applies section 13402(e)(4) of the HITECH Act to Part 2 records), the Secretary must post a list of breaches of unsecured Part 2 records affecting 500 or more individuals. The following breaches have been reported to the Secretary:
